Better Ways to Communicate Hospital Data to Physicians

There are multiple explanations as to why health system administrators have been slow to share data with physicians. The two most common challenges are difficulty obtaining accurate, clinically meaningful data and lack of knowledge among administrators about communicating data.

When it comes to obtaining accurate, meaningful data, the reality is that many health systems do not know where to start. Between disparate data-collection systems, varied physician needs, and an overwhelming array of available clinical evidence, it can be daunting to try to develop a robust, yet streamlined, approach.

As for the second problem, many administrators have simply not been trained to effectively communicate data. Health system leaders tend to be more comfortable talking about costs, but physicians generally focus on clinical outcomes. As a result, physicians frequently have follow-up questions that administrators interpret as pushback. It is important to understand what physicians need.

Determine the appropriate amount and type of data to share. Using evidence and data can foster respectful debate, provide honest education, and ultimately align teams.

Physicians are driven by their desire to improve patient outcomes and therefore want the total picture. This includes access to published evidence to help choose cost-effective drug and device alternatives without hurting outcomes. Health system administrators need to provide clinicians with access to a wide range of data (not only data about costs). Ensuring that physicians have a strong voice in determining which data to share will help create alignment and trust. A more nuanced value-based approach that accounts for important clinical and patient-centered outcomes (e.g., length of stay, post-operative recovery profile) combined with cost data may be the most effective solution.

While physicians generally report wanting more cost data, not all physicians have the experience and training to appropriately incorporate it into their decision making. Surveyed physicians who have had exposure to a range of cost data, data highlighting clinical variation, and practice guidelines generally found cost data more influential in their selection of drugs and devices, regardless of whether they shared in savings under value-based care models. This was particularly true for more veteran physicians and those with private-practice experience who have had greater exposure to managing cost information.

Health systems can play a key role in helping physicians use cost and quality data to make cost-effective decisions. We recommend that health systems identify a centralized data/analytics department that includes representatives of both quality-improvement teams and technology/informatics to own the process of streamlining, analyzing, and disseminating data.

Compare data based on contemporary evidence-based guidelines. Physicians would like to incorporate reliable data into their decision-making when selecting drugs and devices. In our survey, 54% of respondents reported that it was either “extremely important” or “very important” that hospitals use peer-reviewed literature and clinical evidence to support the selection of medical devices. Further, 56% of respondents said it was “extremely important” or “very important” that physicians be involved in using data to develop clinical protocols, guidelines, and best practices….(More)”.

Positive deviance, big data, and development: A systematic literature review


Paper by Basma Albanna and Richard Heeks: “Positive deviance is a growing approach in international development that identifies those within a population who are outperforming their peers in some way, eg, children in low‐income families who are well nourished when those around them are not. Analysing and then disseminating the behaviours and other factors underpinning positive deviance are demonstrably effective in delivering development results.

However, positive deviance faces a number of challenges that are restricting its diffusion. In this paper, using a systematic literature review, we analyse the current state of positive deviance and the potential for big data to address the challenges facing positive deviance. From this, we evaluate the promise of “big data‐based positive deviance”: This would analyse typical sources of big data in developing countries—mobile phone records, social media, remote sensing data, etc—to identify both positive deviants and the factors underpinning their superior performance.

While big data cannot solve all the challenges facing positive deviance as a development tool, they could reduce time, cost, and effort; identify positive deviants in new or better ways; and enable positive deviance to break out of its current preoccupation with public health into domains such as agriculture, education, and urban planning. Challenges facing social media platforms in conflict prevention in Kenya since 2007: A case of Ushahidi platform

The media can play its important role of being the society’s watch dog of the society, by exposing to the masses what is essential but hidden, however the same media may also be used to mobilize masses to violence. Social media can therefore act as a tool for widening the democratic space, but can also lead to destabilization of peace.

The aim of the study was to establish the challenges facing social media platforms in conflict prevention in Kenya since 2007: a case of Ushahidi platform in Kenya. The paradigm that was found suitable for this study is Pragmatism. The study used a mixed approach. In this study, interviews, focus group discussions and content analysis of the Ushahidi platform were chosen as the tools of data collection. In order to bring order, structure and interpretation to the collected data, the researcher systematically organized the data by coding it into categories and constructing matrixes. After classifying the data, the researcher compared and contrasted it to the information retrieved from the literature review.

The study found that One major weak point social media as a tool for conflict prevention is the lack of ethical standards and professionalism for the users. It is too liberal and thus can be used to spread unverified information and distorted facts that might be detrimental to peace building and conflict prevention. This has led to some of the users already questioning the credibility of the information that is circulated through social media. The other weak point about social media as tool for peace building is that it is dependent to a major extent on the access to internet. The availability of internet in low units doesn’t necessarily mean cheap access. So over time the high cost of internet might affect the efficiency of the social media as a tool. The study concluded that information credibility is essential if social media as a tool is to be effective in conflict prevention and peace building.

The nature of social media which allows for anonymity of identity gives room for unverified information to be floated around the social media networks; this can be detrimental to the conflict prevention and peace building initiatives. There is therefore need for information verification and authentication by a trusted agent, to offer information appertaining to violence, conflict prevention and peace building on the social media platforms. The study recommends that Ushahidi platform should be seen as an agent of social change and should discuss the social mobilization which may be able to bring about. The study further suggest that if we can look at Ushahidi platform as a development agent, can we then take this a step further and ask, or try to find, a methodology that looks at the Ushahidi platform as peacemaking agent, or to assist in the maintenance of peace in a post-conflict thereby tapping into Ushahidi platform’s full potential….(More)”.

Many Around the World Are Disengaged From Politics


Richard Wike and Alexandra Castillo at Pew Research Center: “An engaged citizenry is often considered a sign of a healthy democracy. High levels of political and civic participation increase the likelihood that the voices of ordinary citizens will be heard in important debates, and they confer a degree of legitimacy on democratic institutions. However, in many nations around the world, much of the public is disengaged from politics.

To better understand public attitudes toward civic engagement, Pew Research Center conducted face-to-face surveys in 14 nations encompassing a wide range of political systems. The study, conducted in collaboration with the Center for Strategic and International Studies (CSIS) as part of their International Consortium on Closing Civic Space (iCon), includes countries from Africa, Latin America, Europe, the Middle East and Southeast Asia. Because it does not represent every region, the study cannot reflect the globe as a whole. But with 14,875 participants across such a wide variety of countries, it remains a useful snapshot of key, cross-national patterns in civic life.

The survey finds that, aside from voting, relatively few people take part in other forms of political and civic participation. Still, some types of engagement are more common among young people, those with more education, those on the political left and social network users. And certain issues – especially health care, poverty and education – are more likely than others to inspire political action. Here are eight key takeaways from the survey, which was conducted from May 20 to Aug. 12, 2018, via face-to-face interviews.

Most people vote, but other forms of participation are much less common. Across the 14 nations polled, a median of 78% say they have voted at least once in the past. Another 9% say they might vote in the future, while 7% say they would never vote.

With at least 9-in-10 reporting they have voted in the past, participation is highest in three of the four countries with compulsory voting (Brazil, Argentina and Greece). Voting is similarly high in both Indonesia (91%) and the Philippines (91%), two countries that do not have compulsory voting laws.

The lowest percentage is found in Tunisia (62%), which has only held two national elections since the Jasmine Revolution overthrew long-serving President Zine El Abidine Ben Ali in 2011 and spurred the Arab Spring protests across the Middle East.

Attending a political campaign event or speech is the second most common type of participation among those surveyed – a median of 33% have done this at least once. Who represents the human in the digital age?

Auerswald talks about the long history of humans developing codeas a mechanism by which to create and regulate activities and markets.[1] We have Codes of Practice, Ethical Codes, Building Codes, and Legal Codes, just to name a few.

Each and every one of these is based on the data of human behaviour, and that data can now be collected, analysed, harvested and repurposed as never before through the application of intelligent machines that operate and are instructed by algorithms. Anything that can be articulated as an algorithm—a self-contained sequence of actions to be performed—is now fertile ground for machine analysis, and increasingly machine activity.

So, what does this mean for us humans who, are ourselves a conglomeration of DNA code? I have spent many years thinking about this. Not that long ago my friends and family tolerated my speculations with good humour, but a fair degree of scepticism. Now I run workshops for boards and even my children are listening far more intently. Because people are sensing that the invasion of the ‘Social Machine’ is changing our relationship with such things as privacy, as well as with both ourselves and each other. It is changing how we understand our role as humans.

The Social Machine is the name given to the systems we have created that blur the lines between computational processes and human input, of which the World Wide Web is the largest and best known example. These ‘smart machines’ are increasingly pervading almost every aspect of human existenceand, in many ways, getting to know us better than we know ourselves.

So who stands up for us humans? Who determines how society will harness and utilise the power of information technologies whilst ensuring that the human remain both relevant and important?…

Philanthropists must equip themselves with the knowledge they need in order to do good with digital

Consider the Luddites as they smashed the looms in the early 1800s. Their struggle is instructive because they were amongst the first to experience technological displacement. They sensed the degradation of human kind and they fought for social equality and fairness in the distribution of the benefits of science and technology to all. If knowledge is power, philanthropy must arm itself with knowledge of digital to ensure the power of digital lies with the many and not the few.

The best place to start in understanding the digital world as it stands now is to begin to see the world, and all human activities, through the lens of data and as a form of digital currency. This links back to the earlier idea of codes. Our activities, up until recently, were tacit and experiential, but now they are becoming increasingly explicit and quantified. Where we go, who we meet, what we say, what we do is all being registered, monitored and measured as long as we are connected to the digital infrastructure.

A new currency is emerging that is based on the world’s most valuable resource: data. It is this currency that connects the arteries and capillaries, and reaches across all disciplines and fields of expertise. The kind of education that is required now is to be able to make connections and to see the opportunities in the interstice between policy and day-to-day reality.

The dominant players in this space thus far have been the large corporations and governments that have harnessed and exploited digital currencies for their own benefit. Shoshana Zuboff describes this as the ‘surveillance economy’. But this data actually belongs to each and every human who generates it. As people begin to wake up to this we are gradually realising that this is what fuels the social currency of entrepreneurship, leadership and innovation, and provides the legitimacy upon which trust is based.

Trust is an outcome of experiences and interactions, but governments and corporations have transactionalised their interactions with citizens and consumer through exploiting data. As a consequence they have eroded the esteem with which they are held. The more they try to garner greater insights through data and surveillance, the more they alienate the people they seek to reach.

If we are smart what we need to do, as philanthropists, is to understand the fundamentals of data as a currency and integrate this in to each and every interaction we have. This will enable us to create relationships with the people that are based on the authenticity of purpose, supported by the data of proof. Yes, there have been some instances where the sector has not done as well as it could and betrayed that trust. But this only serves as a lesson as to how fragile the world of trust and legitimacy are. Can the UN Include Indigenous Peoples in its Development Goals?: There’s An App For That

The proposal was a web application called “Indigenous Navigator,” designed to enable native peoples to monitor their rights from within their communities. The project is extremely seductive: why rely on the General Assembly to represent Indigenous peoples when they can represent themselves—remotely and via cutting-edge data-collecting technology? Could an app be the answer to over a decade of failed attempts to include Indigenous peoples in the international body?

The web application, which officially launched in 11 countries early this year, is comprised of four “community-based monitoring tools” that are designed to bridge the gap between Indigenous rights implementation and the United Nations goals. The toolbox, which is available open-access to anyone with internet, consists of: a set of two impressively comprehensive surveys designed to collect data on Indigenous rights at a community and national level; a comparative matrix that illustrates the links between the UN Declaration on Indigenous Rights and the UN development goals; an index designed to quickly compare Indigenous realities across communities, regions, or states; and a set of indicators designed to measure the realization of Indigenous rights in communities or states. The surveys are divided by sections based on the UN Declaration on the Rights of Indigenous Peoples, and include such categories as cultural integrity, land rights, access to justice, health, cross-border contacts, freedom of expression and media, education, and economic and social development. The surveys also include tips for methodological administration. For example, in questions about poverty rates in the community, a tip provided reads: “Most people/communities have their own criteria for defining who are poor and who are not poor. Here you are asked to estimate how many of the men of your people/community are considered poor, according to your own criteria for poverty.” It then suggests that it may be helpful to first discuss what are the perceived characteristics of a poor person within the community, before answering the question….(More)”.

Emerging Labour Market Data Sources towards Digital Technical and Vocational Education and Training (TVET)

The ideal situation is a world in which labour market participants engage in intelligent strategic behavior in an informed, fair and sophisticated manner.

Labour market data captures transactions within labour market processes. In order to successfully capture such data, we need to understand the specifics of these market processes. Designing an ecosystem of labour market matching facilitators and rules of engagement for contributing to a lean and streamlined Logistics Management and Information System (LMIS) is the best way to create Big Data with context relevance. This is in contrast with pre-existing Big Data captured by global job boards or social media for which relevance is limited by the technology access gap and its variations across the developing world.

Network effects occur in technology and job facilitation, as seen in the developed world. Managing and instigating the right network effects might be crucial to avoid fragmented stagnation and inefficiency. This is key to avoid throwing money behind wrong choices that do not gain traction.

A mixed mode approach is possibly the ideal approach for developing countries. Mixing offline and online elements correctly will be crucial in bridging the technology access gap and reaping the benefits of digitisation at the same time.

Properly incentivising the various entities is critical for progression, and more specifically the private sector, which is significantly more agile and inventive, has “skin in the game” and a long-term commitment to the conditions in the field, has intimate knowledge of how to solve the the technology gap and brings a better understanding of the particular ambient context they are operating in. Managing expectations and creating incentives for the various stakeholders will be crucial in establishing digitally supported TVET. The New York City Business Atlas: Leveling the Playing Field for Small Businesses with Open Data

The New York City Business Atlas, conceived by the Mayor’s Office of Data Analytics (MODA) and the Department of Small Business Services, is designed to alleviate that information gap by providing a public web-based tool that gives small businesses access to high-quality data to help them decide where to establish a new business or expand an existing one. e tool brings together a diversity of data, including business-fling data from the Department of Consumer Affairs, sales-tax data from the Department of Finance, demographic data from the census, and traffic data from Placemeter, a New York City startup focusing on real-time traffic information.

The initial iteration of the Business Atlas made useful and previously inaccessible data available to small-business owners and entrepreneurs in an innovative manner. After a few years, however, it became clear that the tool was not experiencing the level of use or creating the level of demonstrable impact anticipated. Rather than continuing down the same path or abandoning the effort entirely, MODA pivoted to a new approach, moving from the Business Atlas as a single information-providing tool to the Business Atlas as a suite of capabilities aimed at bolstering New York’s small-business community.

Through problem- and user-centered efforts, the Business Atlas is now making important insights available to stakeholders who can put it to meaningful use—from how long it takes to open a restaurant in the city to which areas are most in need of education and outreach to improve their code compliance.
